For pregnancy, abortion-related and post-abortion scans, the centre requires local ID proof, a doctor’s prescription/referral and two passport-size photographs. These are the centre’s stated documentation requirements; legal requirements can vary by applicable law and authority.
List of documents need for all pregnancy scans , post abortion / abortion related scans, suspected pregnancy scans : - Patient's aadhar with local address , Recent Registered local Doctor's prescription slip with valid doctor's signature and stamp (not older than 1 month), two passport size photographs, Husband's aadhar card may be asked in special cases if patient's ID not local. Accuscan centre's referral form is not valid.
*Timing is general guidance. NT/NB and anomaly-scan timing depends on gestational age, early scan fetal length (Crown rump length-CRL) and clinical circumstances. Ultrasound is a screening/diagnostic imaging tool as appropriate, but no scan detects every abnormality.
